In such cases, we will conduct an investigation without delay to the extent necessary to achieve the purpose of use, and based on the results, correct, add, or delete the personal information to the extent permitted by the Personal Information Protection Act.

  1. ① After receiving your request, we will mail you the designated request form, the “Request for Disclosure, etc. of Personal Information Subject to Disclosure.” Please mail the completed request form, a document verifying your identity if the request is made by proxy, and a postal money order for the handling fee (only for requests for notification of purpose of use and disclosure), to the Personal Information Inquiry Desk listed below.

If the requester is a proxy, please enclose documents verifying their identity as the proxy and the proxy themselves.
